QEval includes evaluation forms that help QA managers to monitor and evaluate the performance of agents for handling the calls. The forms page enables one to create, edit, delete, activate, deactivate or preview the form.
To access this functionality, select Manage > Forms as shown below:
The Design Form page will be displayed as shown below:
The left-hand side panel displays the following:
- Client Name: Select the required name of the client from the dropdown list for whom one can view the desired form.
- All Program: Displays a list of programs based on a particular client selected from the Client Name dropdown list.
Next to the program name, the number indicates the total number of forms that a program has.
My Evaluation Forms for program table displays a list of evaluation forms and their details based on the Client Name and Program selected from the left-hand side panel. The table displays the following details of all the forms:
- Name: Displays the name of the form.
- Status: Displays the status of the form.
- Design: Enables to view the design of the form.
- Active: The selected checkbox depicts that the form is active. To disable it, unselect this checkbox.
- Created By: Displays the name of the user who created the form.
- Created On: Displays the date and time of when the form was created.
- Last Updated By: Displays the name of the user who updated the existing form.
- Last Updated Date: Displays the date of when the form was lastly updated.
- Disable: Enables to disable the form by selecting this option.
- Action: Two action icons will be displayed – Edit and Delete.
From My Evaluation Forms for program, one can view the design of the form, activate or deactivate the form, and edit or delete any particular form.
